1) Company/Organization must provide a private location for the mascot member to prepare for the appearance.
2) Prices are inclusive of VAT and may change without prior notice.
3) All fees must be paid at least two (2) days prior to the scheduled appearance. If appearance fee is not paid by this time, the appearance will be canceled without notice.
This form is required for scheduling any appearance. I acknowledge that I have read and understand the policies related to booking an appearance with the blaqi mascot. I further understand that by completing this form, I am not guaranteed that this request will be fulfilled. I will be notified by phone or email of approval or denial of this appearance.
